Cardiac magnetic resonance predictors for successful primary biventricular repair of unbalanced complete common atrioventricular canal

Background:
Patients with unbalanced common atrioventricular canal can be difficult to manage. Surgical planning often depends on pre-operative echocardiographic measurements. We aimed to determine the added utility of cardiac MRI in predicting successful biventricular repair in common atrioventricular canal.
Methods:We conducted a retrospective cohort study of children with common atrioventricular canal who underwent MRI prior to repair. Associations between MRI and echocardiographic measures and surgical outcome were tested using logistic regression, and models were compared using area under the receiver operator characteristic curve.
Results:We included 28 patients (median age at MRI: 5.2 months). The optimal MRI model included the novel end-diastolic volume index (using the ratio of left ventricular end-diastolic volume to total end-diastolic volume) and the left ventricle–right ventricle angle in diastole (area under the curve 0.83, p = 0.041). End-diastolic volume index ≤ 0.18 and left ventricle–right ventricle angle in diastole ≤ 72° yield a sensitivity of 83% and specificity of 81% for successful biventricular repair. The optimal multimodality model included the end-diastolic volume index and the echocardiographic atrioventricular valve index with an area under the curve of 0.87 (p = 0.026).
Conclusions:Cardiac MRI can successfully predict successful biventricular repair in patients with unbalanced common atrioventricular canal utilising the end-diastolic volume index alone or in combination with the MRI left ventricle–right ventricle angle in diastole or the echocardiographic atrioventricular valve index. A prospective cardiac MRI study is warranted to better define the multimodality characteristic predictive of successful biventricular surgery.

Objectives
This study aims to explore seriously ill patients’ experiences during goals-of-care discussions and perspectives of end-of-life (EOL) decision-making in the Middle Eastern country of Jordan.
MethodsThis is a qualitative descriptive study with semi-structured, one-on-one interviews. Settings were 2 large hospitals in Jordan. Patients were a purposeful sample of 14 Arabic-speaking adults who were seriously ill and hospitalized with palliative care needs.
ResultsConventional content analysis identified 4 main themes: perceived suffering during serious illness, attitudes toward discussing EOL decision-making, goals of care and preferences for EOL, and actions to enhance EOL decision-making. Disease and treatment burdens and concerns about life, family, and death were sources of suffering during serious illness. What matters most to patients at EOL were alleviating suffering and getting support from family, friends, and care providers. Although patients expressed reluctance and inaction toward EOL decision-making due to uncertainties, lacking awareness, and assumptions of fear, their potential goals of care were to live longer, be with their families, and die with dignity.
Significance of resultsJordanians and culturally similar Arabs could benefit from goals-of-care discussions. The proper, culturally sensitive implementation of goals-of-care discussions in Arab populations with similar cultural norms requires raising public awareness and clarifying the legitimacy of goals-of-care discussions, preparing patients and their families for the discussions, and considering individual variations in handling the discussions.
Dietary quality of school meals and packed lunches: a national study of primary and secondary schoolchildren in the UK

Objective:
School lunches represent a key opportunity to improve diets and health of schoolchildren. No recent nationally representative studies have examined the nutritional differences between school meals and packed lunches in the UK. This study aimed to characterise and compare the nutritional quality of school meals and packed lunches among primary and secondary school-age children.
Design:A pooled cross-sectional analysis of the UK’s National Diet and Nutrition Survey (2008–2017).
Setting:United Kingdom.
Participants:3001 children (aged 4–16 years) who completed a 3/4-d food diary which recorded meal type (school meal/packed lunch). Multivariable logistic regression models assessed associations of meeting food and nutrient recommendations by meal type. Analyses were stratified by academic key stages (KS).
Results:KS-1 (4–7 years) and 2 (8–11 years) children consuming school meals were more likely to meet minimum recommendations for vegetables, protein-rich foods and fibre, and not exceed maximum recommendations for salt, savoury and sweet snacks compared with pupils consuming packed lunches. However, in KS-3 (12–14 years) and 4 (14–16 years), these effects were reduced. As children aged, the median weight of fruits, vegetables, protein-rich foods and dairy products consumed typically decreased for both school meals and packed lunches, and generally an increasing proportion of school meals contained sweet and savoury snacks.
Conclusion:These findings suggest school meals are nutritionally superior to packed lunches but are not yet optimal. Quality declined at higher KS. Actions to improve lunches of primary and secondary schoolchildren across the UK are needed, with attention to KS-3 and 4 in secondary schools.

We describe an ultra-wide-bandwidth, low-frequency receiver recently installed on the Parkes radio telescope. The receiver system provides continuous frequency coverage from 704 to 4032 MHz. For much of the band ( ${\sim}60\%$ ), the system temperature is approximately 22 K and the receiver system remains in a linear regime even in the presence of strong mobile phone transmissions. We discuss the scientific and technical aspects of the new receiver, including its astronomical objectives, as well as the feed, receiver, digitiser, and signal processor design. We describe the pipeline routines that form the archive-ready data products and how those data files can be accessed from the archives. The system performance is quantified, including the system noise and linearity, beam shape, antenna efficiency, polarisation calibration, and timing stability.
The perceived influence of cost-offset community-supported agriculture on food access among low-income families

Objective
To examine perspectives on food access among low-income families participating in a cost-offset community-supported agriculture (CO-CSA) programme.
DesignFarm Fresh Foods for Healthy Kids (F3HK) is a multicentre randomized intervention trial assessing the effect of CO-CSA on dietary intake and quality among children from low-income families. Focus groups were conducted at the end of the first CO-CSA season. Participants were interviewed about programme experiences, framed by five dimensions of food access: availability, accessibility, affordability, acceptability and accommodation. Transcribed data were coded on these dimensions plus emergent themes.
SettingNine communities in the US states of New York, North Carolina, Washington and Vermont.
SubjectsFifty-three F3HK adults with children.
ResultsCSA models were structured by partner farms. Produce quantity was abundant; however, availability was enhanced for participants who were able to select their own produce items. Flexible CSA pick-up times and locations made produce pick-up more accessible. Despite being affordable to most, payment timing was a barrier for some. Unfamiliar foods and quick spoilage hindered acceptability through challenging meal planning, despite accommodations that included preparation advice.
ConclusionsAlthough CO-CSA may facilitate increased access to fruits and vegetables for low-income families, perceptions of positive diet change may be limited by the ability to incorporate share pick-up into regular travel patterns and meal planning. Food waste concerns may be particularly acute for families with constrained resources. Future research should examine whether CO-CSA with flexible logistics and produce self-selection are sustainable for low-income families and CSA farms.

We present the results of two 2.3 μm near-infrared (NIR) radial velocity (RV) surveys to detect exoplanets around 36 nearby and young M dwarfs. We use the CSHELL spectrograph (R ~ 46,000) at the NASA InfraRed Telescope Facility (IRTF), combined with an isotopic methane absorption gas cell for common optical path relative wavelength calibration. We have developed a sophisticated RV forward modeling code that accounts for fringing and other instrumental artifacts present in the spectra. With a spectral grasp of only 5 nm, we are able to reach long-term radial velocity dispersions of ~20–30 m s−1 on our survey targets.

Echinoderms are well represented in nearshore hard-bottom (< 100 m depth) habitats along the Antarctic Peninsula where they are presumably important contributors to benthic production, carbon flow, and determinants of community structure. The present study assesses the densities of echinoderms at shallow depths (2–15 m) at five sampling sites within three kilometres of Anvers Island on the central western Antarctic Peninsula. The asteroids Odontaster validus, Granaster nutrix, Lysasterias perrieri and Adelasterias papillosa, two ophiuroids in the Amphiuridae, the holothuroids Psolicrux coatsi and Psolus carolineae and one representative of the Cucumaridae, and the regular echinoid Sterechinus neumayeri were enumerated. Mean total echinoderm densities were high (34.9 individuals m-2) and ranged from 21.9 individuals m-2 for asteroids to 2.7 individuals m-2 for holothuroids. With the exception of a positive relationship between the abundance of the regular echinoid Sterechinus neumayeri and the biomass of the brown alga Himanthothallus grandifolius, no significant relationships were found between the abundance of asteroids, ophiuroids, or holothuroids and two species of brown algae or three algal ecotypes. The present study indicates nearshore hard-bottom echinoderms are important in the carbon cycle and their inherent vulnerability to ocean acidification may have community-level impacts.
A review of the literature on multiple symptoms, their predictors, and associated outcomes in patients with advanced cancer

Objective:
The findings from several studies suggest that palliative care patients with advanced cancer experience multiple symptoms, and that these symptoms may be related to demographic and clinical factors as well as to patient outcomes. However, no systematic review has summarized the findings from studies that assessed multiple symptoms, predictors, and outcomes in these patients. The purposes of this review, focused on palliative care patients with advanced cancer, are to: 1) describe the relationships among multiple symptoms; 2) describe the predictors of multiple symptoms; and 3) describe the relationships between multiple symptoms and patient outcomes.
Method:Comprehensive literature searches were completed using the following databases: PubMed, Cumulative Index to Nursing and Allied Health Literature, and PsychInfo. The key words: cancer or advanced cancer or neoplasm, AND palliative care or terminal care or hospice or end-of-life, AND symptoms or multiple symptoms or symptom clusters were combined.
Results:Twenty-two studies met the inclusion criteria and examined at least one of our purposes. The majority of these studies were descriptive and used one of four common symptom assessment scales. Fifty-six different signs and symptoms were evaluated across various dimensions (i.e., prevalence, severity, distress, frequency, control). Pain, dyspnea, and nausea were the only symptoms measured in all 22 studies. Relationships among concurrent symptoms were examined in nine studies. Relationships among symptoms and predictors (i.e., demographics, cancer type, healthcare delivery environment) were examined in seven studies. Relationships among symptoms and outcomes (i.e., functional status, psychological status, quality-of-life, survival time) were examined in 14 studies. Significant methodological variation was found among these studies.
Significance of results:It is difficult to draw conclusions about the relationships among multiple symptoms, predictors, and outcomes due to the heterogeneity of these studies. Future research is needed to determine which symptoms and symptom dimensions to assess in order to better understand how multiple symptoms relate to each other as well to as predictors and outcomes in palliative care patients with advanced cancer.
Characterization of bismuth telluride aerogels for thermoelectric applications

Refrigeration, air conditioning, and other cooling requirements in buildings, industry, and transportation sectors account for about 10 quads of U.S. primary energy consumption. Therefore, advanced technologies for space cooling in buildings and vehicles – as well as for refrigeration in residential, commercial, and industrial applications – that are more energy efficient, avoid net direct greenhouse gas emissions, reduce lifecycle costs, and can impact large markets are needed. Although current technologies are reaching their efficiency limits, thermoelectric (TE) materials can be used for cooling applications and have potential for significant improvements. Compared to traditional bulk phase TE materials, literature results suggest that nanometer-scale materials allow additional opportunities to improve the efficiency of TE materials. Aerogels are one type of nano-material that offers opportunities to increase the efficiency of TE materials by controlling particle size, particle composition and by reducing the thermal conductivity. Bismuth telluride (Bi2Te3) is the most studied TE material and our objective was to produce bismuth telluride aerogels with controlled microstructures and thermal conductivities to increase the TE figure of merit. Aspen Aerogels developed a novel synthesis method to prepare Bi2Te3 aerogels using the principles of colloidal chemistry and sol-gel chemistry. The reaction conditions were investigated and optimized so that gels could be obtained at low reaction temperatures. The gels were aged and dried using supercritical CO2. The aerogels were characterized by BET, XRD, and SEM. The best aerogels were hot pressed and Seebeck coefficients were determined. The synthetic approach developed and the properties of the aerogels will be presented and compared with Bi2Te3 aerogels and materials prepared by other methods.

Background
Subtle formal thought disorders are difficult to quantify. Their relationship to florid thought disorder is unknown.
AimsTo assess the interrater reliability, sensitivity and factor structure of a new assessment instrument, the Thought and Language Index (TLI), and to determine if minor aberrations detectable in the speech of healthy individuals are related to the more severe formal thought disorders characteristic of schizophrenia.
MethodInterrater reliability was evaluated by determining the intraclass correlation for the ratings by five assessors. Factor analysis of the TLI scores of 87 patients was performed, and TLI scores in matched patients and controls were compared.
ResultsThe intraclass correlation was good for individual TLI items, and excellent for sub-scale scores. Factor analysis identified three groups of approximately orthogonal disorders. Mild speech aberrations were observed in healthy participants and in patients with schizophrenia. The prevalence of mild aberrations was correlated with the prevalence of definite formal thought disorders.
ConclusionsThe TLI is reliable and capable of detecting subtle disorders. Some mild aberrations occurring in the speech of healthy individuals appear to be attenuated forms of the florid disorders characteristic of schizophrenia.

This report summarises the work of the third season of the Fezzan project which took place in January 1999. The main environmental findings of the project team of specialist geographers are providing confirmation of dramatic climatic and environmental change over the last 100,000 years and give more precise dates for some of these changes. The excavations in Old Germa (ancient Garama) have continued through Islamic levels, with elements of five main phases of buildings now having been recorded. Additional standing structures, including one of Germa's main mosques, have been surveyed. Field survey around Germa has revealed further new settlement sites of prehistoric, Garamantian and Islamic date. Of particular importance is a series of lithic and pottery scatters relating to neolithic occupation along the edge of the Ubari Sand Sea, to the north of Germa. Further investigation of the irrigation channels (foggaras) has revealed significant new information about their size, construction and probable date. The report concludes with a brief preliminary analysis of changing settlement patterns over time.
